(Version 1.0, 6/30/2026)
Thank you for considering sharing your experience. Your contribution can help affected individuals and families feel less alone, raise public awareness of AI-related harm, and support The Human Line Project’s work to better understand and prevent it.
Before you submit, please read this statement. By submitting your response, you confirm that you understand and agree to the following.
What you’ll be sharing
When you submit, you may provide:
- Whether you are reporting for yourself or for a loved one
- A list of harms or impacts you or your loved one experienced or observed
- An optional written testimonial, in your own words
- Your email address
Please do not include real names, addresses, phone numbers, private messages, or other details that could identify you or anyone else in the harms list or testimonial. This form is intended to collect limited, non-identifying information about the impact of AI-related harm. If identifying details are included in your testimonial, The Human Line Project may remove or alter them before any wider use.
You can share your own experience, or, if you are a family member, friend, caregiver, or dependent, your account of what happened to someone you know. The permissions you give apply to your account only.
How your information may be used
You keep ownership of your testimonial. By submitting this form, you give The Human Line Project non-exclusive, royalty-free permission to collect, store, review, edit, excerpt, anonymize, and use your submission for public awareness, education, advocacy, research-support, and related organizational purposes.
Your separate answer to the publication question controls whether The Human Line Project may share an anonymized, sanitized version of your testimonial publicly, including on its website or in publications to help raise awareness about this issue.
If you give permission to share your testimonial publicly, The Human Line Project may make light edits for clarity, length, and privacy, without changing the meaning of what you shared. The Human Line Project will not intentionally publish your name, email address, contact details, or other identifying information.
If you do not give permission to share your testimonial publicly, The Human Line Project will not publish your testimonial. We may still use the non-identifying information you submit internally to understand patterns of AI-related harm and support our public awareness, education, advocacy, research-support, and related organizational work.
Your rights
You stay in control of your submission. At any time, you can:
- Withdraw your permission by emailing us. We will stop using your submission going forward as soon as reasonably possible.
- Ask us to stop sharing your testimonial further, if you previously gave permission for public sharing, so it is not reposted or redistributed by The Human Line Project going forward.
- Ask to see what we hold about you, if anything.
- Correct anything that is wrong.
- Ask us to delete or further anonymize your submission.
Withdrawal applies going forward only and will not affect uses that have already happened. If your testimonial has already been shared publicly with your permission, we cannot recall copies that other people have made or shared. Aggregated or anonymized material already used in reports, presentations, or public materials may not be possible to remove.
What you confirm when you submit
By submitting this form, you confirm that:
- You are 18 or over.
- You have read and understood this consent statement.
- The information you are sharing is your own to share.
- You have not knowingly included identifying details about other people without their agreement.
- Nothing in your submission is knowingly false or intended to harm another person’s reputation.
- You understand that permission to publish an anonymized, sanitized version of your testimonial is controlled by your separate answer to the publication question on the form.
